NEBnow® Freezer Program Registration

Please provide the details requested below to register for the NEBnow® Kiosk.

Step One: Complete the form below. Please wait 24 hours for your application to be reviewed.

Step Two: Once approved you will receive an email from NEB asking you to call our Customer Service and provide your credit card or p-card information.

Your card will be securely stored and applied to your NEBnow account. Once a payment method is recorded, you will receive a welcome email with your NEBnow PIN and purchase instructions.

If you ever need to update your lab card information, please just resubmit this form or contact Freezers@neb.com.
